Screening Health Forms

The following Health forms are designed to be used as part of the comprehensive screening protocol described in the narrative above on page 2. They are to be used in the First 5 CA Special Needs Demonstration Sites.

The Health Survey Form matches Step 1A, (more information on page 4 in the narrative) and the Health Screening Form is to be used to record data from a comprehensive health screening (more information on page 2 in the narrative)

Positive Outcomes for Children with Disabilities and Other Special Needs

A collaborative project of the California Institute on Human Services, Sonoma State University and San Francisco State University-Funded by the Proposition 10 California Children and Families Commission.

The Positive Outcomes project had two phases with two reports produced. The first phase of plan development was for Sonoma State University, California Institute on Human Services, in collaboration with San Francisco State University, to compile information about children with disabilities, synthesize key laws and regulations pertaining to this group, participate in the development of equity principles, review State Commission initiatives, and complete initial logistical work toward a more comprehensive strategic planning process to occur during the fall of 2001. The resulting report may be downloaded here.

In November 2001, the California Institute on Human Services (CIHS) at Sonoma State University (SSU) in cooperation with San Francisco State University (SFSU) facilitated stakeholder regional forums in three locations across California: Fresno, Concord, and Pomona. The purpose of the regional forums was to provide a structured input venue for parents and professionals to inform the California Children and Families Commission (CCFC) about key investment areas that the Commission may consider for California children prenatal to five years with disabilities and other special needs. The results of the forums were distilled into a single set of recommendations by a group of participants selected at each of the three regional forums. This document is available to download here.

The Infant, Preschool, Family Mental Health Initiative

This set of documents was produced through the Infant, Preschool, Family, Mental Health Initiative (IPFMHI). These products, a joint effort of First 5 California, the California Department of Mental Health, and the California Institute of Mental Health, are designed to support First 5 Special Needs Demonstrations Sites, First 5 School Readiness Sites, and other early mental health program efforts across the state.

These three guides offer information and outline structures that can support the development of programs that identify social-emotional developmental delays and disorders, and promote access to prevention, early intervention, and treatment services for children birth to 5 years old.

Also available to you is "The Executive Summary of The California Infant Mental Health Workgroup Report: Recommendations for Screening, Assessment, Service Delivery and Training Related to the Promotion of Infant Social and Emotional Mental Health for All Providers of Education, Health and Human Services to Infants, Toddlers and their Families", which provides recommendations related to the promotion of infant social and emotional health and was the catalyst for the Infant, Preschool & Family Mental Health Initiative.
